Care Resources, a Program of All-inclusive Care for the Elderly (PACE) health plan and care provider based in Grand Rapids, Michigan, recently presented at the National PACE Association (NPA) conference about how it achieved a 40% reduction in the organization’s rate of falls among its medically complex, older adult population. Care Resources had deployed the care.coach technology platform, designed to integrate with the PACE program’s interdisciplinary care team (IDT) and drive a range of healthcare quality and cost outcomes.
Care Resources began deploying the care.coach platform in 2020 during the peak of the COVID-19 pandemic to support the psychosocial needs of Care Resources participants affected by social isolation and disrupted access to care. Previous research published by Stanford University has shown that the care.coach Avatar™ significantly reduces loneliness among older adults, and feedback from Care Resources participants was highly positive.

The care.coach program doubled in scale through 2021, and in 2022, the Care Resources Rehabilitation Department led a fall prevention quality improvement project with care.coach, configuring customized exercise programming for an at-risk group of sixteen participants. Exercises were delivered through a combination of one-on-one coaching from the care.coach Avatar™ and group sessions using care.coach Video Visits™, and were combined with a variety of other coaching from each participant’s avatar. Over one year, 6,401 such fall prevention tasks were executed through the care.coach platform, resulting in a 40% reduction in falls. The average PACE participant has 7.9 diagnosed chronic conditions, and because of a variety of other evidence-based, turn-key care.coach coaching and care coordination protocols, Care Resources also observed benefits to medication adherence and reductions in inpatient stays, emergency department visits, anxiety, and depression.

At the 2022 NPA conference, Care Resources and care.coach announced a strategic relationship to provide at least 50% of Care Resources participants with care.coach services. By integrating with PACE IDT workflows, care.coach will improve the efficacy of care plan interventions, while supporting key organizational initiatives to reduce unnecessary healthcare utilization and improve coordination of care.
